1. What you're agreeing to
By participating in classes at Bloc Pilates (operated by [FULL LEGAL / TRADING NAME], ABN [ABN]), you confirm you’ve read and understood this waiver, you’re taking part voluntarily, and you accept the risks described below.
2. About reformer Pilates & its risks
Bloc runs a hard, athletic style of reformer Pilates. The reformer is a spring-loaded machine with a moving carriage, straps and a footbar. Like any physical exercise, it carries inherent risks that can’t be completely removed, including:
- ·muscle soreness, strain, sprains or tears;
- ·slips, trips or falls, and pinch points around the moving carriage, springs and straps;
- ·aggravation of a pre-existing injury or condition;
- ·in rare cases, more serious injury or a cardiac event.
3. Assumption of risk & risk warning
This is a risk warning for the purposes of the Civil Liability Act 2002 (WA). Reformer Pilates is a recreational activity with obvious and inherent risks. You acknowledge those risks and choose to participate anyway, taking responsibility for your own participation and for exercising within your limits.
4. Health screening & your declaration
Before your first class you’ll complete a pre-exercise health screen (based on the industry-standard [Adult Pre-Exercise Screening System (APSS)]). You declare that:
- ·the information you give us is true and complete;
- ·you’ve disclosed any injuries, medical conditions, medications, pregnancy or recent surgery;
- ·if your health changes, you’ll update us before your next class;
- ·where your screen flags it, you’ll obtain medical clearance from your doctor before taking part.
5. Your responsibilities in class
- ·Follow your instructor’s directions and the studio’s safety rules.
- ·Wear grip socks and appropriate clothing.
- ·Work within your own limits and stop if you feel pain, dizziness or distress — and tell your instructor.
- ·Don’t take part while affected by alcohol or drugs.
6. Hands-on cueing & adjustments
From time to time your instructor may use light hands-on cues to guide your alignment. You can decline hands-on adjustments at any time, for any reason — just let us know. [Add a tick-box for hands-on consent at sign-up.]
7. Release of liability
To the extent permitted by law, you release Bloc Pilates and its instructor from liability for harm resulting from the obvious and inherent risks of participating, where we have taken reasonable care. This waiver applies to the consumer guarantees for recreational services as allowed by section 139A of the Australian Consumer Law.
Important limits: nothing in this waiver excludes liability that can’t be excluded by law, and it does not cover injury caused by our reckless conduct or gross negligence. Your rights under the Australian Consumer Law still apply.
8. Not medical advice
Our classes and any guidance we give are for general fitness and are not medical advice, diagnosis or treatment. If in doubt, consult your doctor or an allied health professional.
9. Under 18s
[If minors are permitted], a parent or guardian must read and accept this waiver on the participant’s behalf and complete the health screen. If Bloc operates as an adults-only (18+) studio, this section can be removed.
10. Media consent (optional)
We sometimes capture photos or video in class. Media consent is handled separately and is always optional — see our Code of Conduct. You can opt out any time.
